Welcome to Pointe South Middle School! We will host face-to-face registration to students new to Clayton County on July 20, 2021 through July 30, 2021 from 8:30 until 12:00, and 1:00 until 3:30. Please be prepared to follow CDC guidelines by wearing a mask and practicing social distancing. To limit the number of people in the building, only the person enrolling the student will be allowed. The following documents are needed for complete enrollment:
· Birth Certificate
· Guardianship or custody papers if not the birth parent
· Student Social Security Card
· Georgia Immunization Certificate Form (Form 3231)
· Georgia Eye, Ear and Dental Certificate (Form 3300)
· Withdrawal records from previous school including last grade placement, report card, transcript, IEP, 504, gifted verification, discipline history
· Parent ID
· Current verification of residency (two proofs in the parent/guardian’s name)- lease or mortgage statement, utility bill (gas, electric, water)
